Diseases of the Aorta Flashcards
What is a collagen syndrome that can effect the Aorta?
Marfan Syndrome
Ehlers Danlohs syndrome
What are some possible imaging modalities for diagnosing AAA?
Chest XR
ULtrasound
MRI
What are risk factors for AAA?
Atherosclerosis and smoking
Male and Advanced age, dyslipidemia and Fam hist
Genetic Predisposition
Infectious
What are the Sx of AAA?
Incidental finding Low abd/back pain TRIAD: Pain, Hypotension, Pulsitile Abdominal mass Abdominal Bruit Sxfrom space occupying lesion
What is the medical Tx of AAA?
Smoking Cessation Lipid Control BP Control Aspirin BB ACE-I
What are the Surgical Tx of AAA? GRASS
High momortality for ruptured AAA Rupture Symptomatic > 5.5 cm in males 5 cm in Females Rapid Expansion >.5cm in 6 mon AAA 4-5.4 require imaging every 6-12 mon
Who can you screen for AAA?
Men 65-75 yo who have ever smoked are good for 1 screening
What are the features of Thoracic Aortic Aneurism?
Less common than AAA Involve Aortic Root, ascending Aorta(most common) Aortic Arch Descending aorta(2nd most common)
What is the Medical Tx for TAA?
Control risk factors:
BP Control
dP/dt
HR
What are the Acute Aortic Syndromes?
Aortic Dissection Intramural Hematoma Penetrating Aortic Ulcer Rapid Aneurism Expansion Aortic Rupture
What is the Classification of Aortic Dissection?
60% DeBakey 1 Ascending and Descending
10-15% DeBakey 2 Just the Ascending
25-30% DeBakey 3. Just the Descending
Which of the 3 types are the most dancerous and urgent?
The ones involving the Ascending are more severe because they occurr at a higher pressure and are more likely to enlarge.
What Lab test cn be used to rule out Acute Dissection?
D-Dimer
What are medical Tx in Acute Dissection of Aorta?
Want to get the HR and BP down
Beta Blockers
Metoprolol, Labetolol, Esmolol, Propanolol
How often does followup occur in Aortic Dissection?
at 1, 3, 6, and 12 months
