What should be done 1st when d/c a pt write orders or resolve problems?

A

Orders should be written 1st because the orders need to be associated with the problem list. Only active problems show up for selection during medication reconciliation if resolve the problem will not be there.

What is the difference between d/c summary and pt instructions

A

D/C is a clinical summary of the pt encounter, read by other clinicians and is a permanent part of the chart
Pt instructions is s list of medications, education and referrals and appointments it is printed and given to the pt
