Definition of diarrhoea
Passage of >3 loose stools per day
Define:
Acute diarrhoea
Persistent diarrhoea
Chronic diarrhoea
Acute: <14 days
Persistent: >14 days
Chronic: >4 weeks
Most common cause of acute diarrhoea
Viral/ bacterial infection
Other causes of acute diarrheoea
Anxiety, drugs, food allergy, acute appendicitis
Causes of chronic diarrhoea
IBS, IBD, coeliac disease, bowel cancer
Causes of bloody diarrhoea (infection/ structural)
Infection: campylobacter, shigella, salmonella, e coli, amoeba
Structural: IBD, polyps, ischaemic colitis
Causes of explosive diarrhoea
Rotavirus, giardia, cholera
Causes of pus in diarrhoea
IBD, diverticulitis, fistula/ abscess
Causes of steatorrhoea
Pancreatic insuffciency or biliary obstruction
Red flags for diarrhoea
Blood in stool Recent hospital treatment or abx use Weight loss Dehydration Nocturnal symptoms
Indications for stool sample
Systemically unwell
Blood or pus
On abx/ PPI
Recent foreign travel
NO SYSTEMIC SIGNS + SYMPTOMS = NO STOOL SAMPLE NEEDED!
Investigations in chronic diarrhoea
FBC, U+Es, LFTs, calcium, B12, folate, ferritin, TFTs, ESR, CRP, test for coeliac disease
What does ORT contain?
Glucose Sodium Potassium Chloride Citrate
