Diaper Rash
A
- irritant contact dermatitis
- only affect areas where diarrhea splashes
- skin folds typically spared
- self-limiting –> episodes approx last 2-3 days

Causes of diaper rash
A
- fecal/urine contact –> viral gastroenteritis –> harsh enzymes in diarhhea
- chemical residue
- chaffing
- antibiotic –> diarhhea as s/e
- Formula more problematic than breast milk

Non-med tx of diaper rash
A
- change diapers as quickly as possible
- keep area clean
Barrier products

Tx of diaper rash
A
- keep clean
- barrier product
- allow air time
- steroid crm

How to tell if diaper rash is yeast infected? How long to tx?
A
- If just diaper rash –> tx > 3 days
- If yest infected, <3 days and:
a) Fiery red
b) Vesicles/satellites
c) skin folds now involved

Oral Thrush and Diaper Rash
A
- oral Thrush - Yeast infection of the mouth –> if have with diaper rash, automatically fungal

Med Tx of Diaper Rash Antifungals
A
- Clotrimazole 1%
- Miconazole 2%
BID 1 week + 1 week after clearing

MD Referral
A
- No improvement in 7 days or resolve in 14 days

Steroids
A
- HC 0.5-1% for 2 days

Uninfected tx of diaper rash
A
- HC 0.5-1% BID X 1-2 days
- Barrier cream 4-5x per day everyday

Infected tx of diaper rash
A
- Anti-fungal cream BID for 14 days
- HC 0.5-1% BID x 1-2 days
- BArrier cream 4-5x per day everyday

A
- Antifungal
- Steroid
- Barrier
- Wait 2-3 mins before each application
- DO not mix anti-fungal and corticosteroids
