Diagnostic imaging for soft tissue pathology Flashcards

1
Q

7yo Eventing Pony
Acutely, severely lame following completion of the cross-country phase
Heat, pain and swelling found during palpation of the metacarpus
Slight hyperextension of the metacarpophalangeal joint whilst walking

Is this lesion best described as “diffuse” or a “core lesion”?

Which soft tissue structure is the focus of injury in this case?

A

Core lesion with, Diffuse pathology in the background,

SDFT

Q

4yo Connemara pony
Lame when brought in from field yesterday
Severe lameness then has improved slightly
Palpable thickening on the lateral aspect of the limb in the proximal third of the metacarpus
Warm, inflamed and pain on palpation

Which structure is involved in this injury?

Q

18yo Hunter
Hunted yesterday
No reported trauma or previous lameness
Sound last night but lameness evident at trot today
Marked effusion as seen in the photograph

Q

How would you describw the lesion in this case?
what is the cause of this finding?

A

Hyperechoic (line, then a shadow behind it)
cause: mineralisation (but could be gas, FB, bone avulsion) (depends on level of lameness and history of trauma)

Q

12yo Dressage Horse
Intermittent lameness responsive to rest
Recent episode 24 hours ago
Moderate lameness, worst when lunging on the soft surface
Palpable thickening over lateral aspect of the distal metacarpus

What is the bright line indicated by the arrow?
what is the soft tissue that is affected?

A

line: surface of the proximal sesamoid bone
soft tisuse: lateral susspensory branch

Q

16yo Dressage Horse
Acute moderate right hind lameness noted yesterday after schooling session
Today sound at walk but lame at trot
No significant findings during palpation of the hind limbs
Which soft tissue is affected?
how else should we evaluate this case?

A

Susspensory:
Right - Susspensory ligament has expanded on the dorsal surface

longitudinal scanning: (see picture), right is bigger and loss of paralelle fibres

Q

11yo Cob used for hacking
Galloped whilst riding yesterday and was immediately lame afterwards
Marked effusion of the tendon sheath
Positive to distal limb flexion

Which structure are within the digital flexor tendon sheath appears to be injured in this case?
what should be done to make this injury easier to visulise?

A

Manica flexoria - should hold the SDFT and DDFT close togeter with no synovial fluid between

contrast radiography - loss of tow line on the dorsal surface - see normal below

Q

18yo Military working horse
Acute hindlimb lameness noted after turnout
Notable fetlock hyperextension when standing
Fluctuant swelling palpable just distal to the calcaneus
Whould you perfrom diagnostic anaesthesia in this case?

Grade the severity of this injury?

Q

20yo riding horse
Recurrent bouts of hindlimb lameness over the previous 2 years
Effusions of the medial femorotibial joint and femeropatellar joints easily palpable
Horse improved 60% to anaesthesia of the medial femorotibial joint

What structure is abnormal in this case?

A

medial meniscus
The medial femoral condyle is also undergoing remodling
