Egosyntonic v Egodystonic
- How individual experiences their dx
- Egosyntonic - part of clt’s view of self and doesn’t see behaviors as a probs, dev really early in life (often traumatic upbringing) and becomes their norm
- Egodystonic - something clt is aware of and doesn’t like or fit into their perception of self, e.g. anxiety, depression, panic dx, complains of symptoms
Paranoid Personality Disorder vs. Delusional Disorder vs. Schizophrenia
Paranoid Personality Dx
- Pervasive, irrational suspicions and mistrust of others, reads into messages and remarks as persecuratory, believes world is hostile, holds grudges, constantly suspects others are “out for them” w/o evidence
- Preoccupied about loyalty of friends, hard to maintain friendships
Delusional Dx
- ONLY delusions - Can be bizarre (e.g. movie star loves me) or nonbizarre (e.g. movie star’s an alien)
*Most likely not on test
Schizophrenia
- e.g. hallucinations, disorg speech, catatonia
- No longer delusional dx
Schizoid Personality Disorder vs. Avoidant Personality Disorder vs. Major Depressive Disorder
- Schizoid and Avoidant - egosyntonic
- MDD - egodystonic
Schizoid Personality Dx
- Loners, e.g. lack interest in social relationships, withdrawn, but not w/ an awareness or longing to connect
- Detachment from social relationships, prefer being alone
- May have some depression b/c of their isolation
- Pervasive, over the course of someone’s life
- Don’t recog this is a problem
Avoidant Personality Dx
- Express a fear of being ridiculed, preoccupation w/ being criticized, loneliness, and social isolation
- Feelings of inadequacy and that ppl don’t like them, avoids interxs for fear of being evaluated negatively
- Strong desire for social relationship, want to be connected to other ppl
- Poor self-image
MDD
- Can see marked change of socialization and don’t like it
Schizotypal Personality Disorder vs. Schizophrenia vs. Schizoid
Schizotypal Personality Dx
- Characterized by off personality, irrational beliefs, and ideas of reference, e.g. obsessed w/ aliens, thinks news headline directly speaks to them, magical thinking, bizarre fantasies
- May have some suspicion
- No psychotic episode
- Lacks friends b/c they’re odd and may dress bizarrely
Schizophrenia
- Psychotic episode
- Schizotypal may be dx first prior to psychotic
Schizoid Personality Dx

Antisocial Personality Disorder vs. Narcissistic Personality Disorder
Antisocial Personality Dx
- Happens prior to 15 yo, pervasive pattern of disregard for violation of others, e.g. law breaking, deception, repeated lying for personal profit or pleasure
- Impulsive, aggressive, irresponsible
- Lack of remorse after they’ve done something wrong
Narcissistic Personality Dx
- Grandiose sense of self importance, e.g. fantasies of success and power, think they’re special, entitled, envious of others, arrogant
- Can go into rage if discounted or put down
- Common dx for DV perpetrators
- Need for admiration
- Lack of empathy
Borderline Personality Disorder vs. Histrionic Personality Disorder
Borderline Personality Dx
- Known for instability in relationships
- Fear of real or imagined rejection and can go to extremes, e.g. recurrent suicidal gestures, self injury
- Black and white thinking. e.g. one min they like you, then they hate you, idealizes thp and then immediately devalue you
- Thp needs to ID disturbances in relationship to other ppl
- Mood instability, emptiness and difficulty controlling rage
Histrionic Personality Dx
- Attention seeking behaviors and want to be center of attention, e.g. seductive dressing, excessive flirting
- Rapid shifting and shallow expression of emotions
- Can be very vain and exaggerated expression of emos
