Diabetes insipidus Flashcards
1
Q
two main types of Diabetes Insipidus
A
- Pituitary (central)
- Nephrogenic
2
Q
ADH/vasopressin2 receptors
Stimuli for release
A
- distal segment of kidney
Stimuli
- osmotic stimulus (inc blood osmolality)
- Volume stimulus (hypovolemia, dec CO)
3
Q
Diagnostic criteria
A
- Hx PU/PD
- Normal PE
- Normal chemistries
- Hyposthenuric < 1.008
4
Q
Rule outs for PU/PD
A
- Diabetes Mellitus
- Cushings
- Chronic renal dz
- Pyo
- Drugs
- Addison’s
5
Q
Diagnostic tests to differentiate DI and psychogenic Polydypsia
A
- Water deprivation test => easiest
- avoid hypertonicity, hypervolemia 6-24 hrs
- don’t let lose more than 3% body weight
- Vasopressin Test
- Combined WD/VT
- ADH trial
6
Q
Differentiate central and nephrogenic DI
A
- Water deprivation test distinguishes DI from PPD
-
Vasopressin test distinguishes Pit from Nephrogenic DI
- if animal responds (concentrates urine) to vasopressin test dx is Pit DI
7
Q
adverse affects of water dep test
A
- Hypertonic encephalopathy
- if dog loses 3-5 % of body weight can become hypernaetremic and show CNS signs
8
Q
DI TX pit
A
- DDAVP
- tabs, SQ, eye drops
- very expensive
9
Q
TX Nephrogenic Diabetes Insipidus
A
- Chlorothiazide
- diuretic
- Low sodium diet
