What is the pathophysiology of gestational diabetes?

A

Hormones produced by placenta (HPL) are anti-insulin = insulin resistance
There is beta-cell hypertrophy during first half of pregnancy so signs of GDM don’t arise until second half of pregnancy

A

24-28 weeks

How is GDM diagnosed?

A

GCT with 50 g glucose, if 1 hour after gluc>130 or 140 then positive
Then GTT - 100 g load glucose and check fasting, and 3 hour glucose (normal = 105/190/165/145

What is management of GDM?

A
diet and exercise (limit carbs and overall caloric intake)
metformin or glyburide
insulin
