- you can see it, but you can’t feel it!

Patch
- large flat discoloration, ≥ 1 cm

- large bump with depth in skin
* keloid scar
Plaque
- large raised flat topped lesion, ≥ 1 cm
- usually scaly
- psoriasis
Vesicle
- small blister, ≤ 1 cm
* herpes
Bulla
- large blister, ≥ 1cm
* bullous pemphigoid
Pustule
- pus filled bump
* acne
Wheal
- raised, smooth topped lesion
- sometimes called “welt”
- urticaria
Scale
- roughness at surface
- represents hyperkeratosis
- psoriasis
Crust
- dried plasma at surface
- represents epithelial disruption
- eczema
Excoriation
•scratched area, often linear
Lichenification
- leathery thickening with accentuated skin markings
* chronically rubbed skin of eczema
Telangiectasia
- dilated capillaries
* sun-damaged skin
Onycholysis
- separation of nail from bed
* onychomycosis
Hyperkeratosis
•thickening of stratum corneum
Parakeratosis
•retained nuclei in stratum corneum
Hypergranulosis
•increase in granular layer
Acanthosis
•thickening of epidermis
Papillomatosis
•mountain and valley pattern in epidermis
Acantholysis
•falling apart of keratinocytes - discohesion
Spongiosis
•intercellular edema - between keratinocytes
Ballooning
•intracellular edema of keratinocytes
Exocytosis
•lymphocytes migrating into epidermis
Ulceration
•complete loss of epidermis
Acral
•distal portions of limbs and head
Dermatomal
•dermatome distribution
Extensor surfaces
•area of skin outside of a joint
Flexural surfaces
•the side of a joint that folds
Intertriginous
•where skin areas touch
