75 yo Pt presents with a plaque on her foot that looks like a coin stuck her with waxy appearance. Is this a malignant or benign presentation? What would you see on histology? What would happen if she had more than one of these coins?

A

She has SEBORRHEIC KERATOSIS. This is a benign squamous proliferation common in the elderly. Characterized by keratin pseudo cysts on histology. If multiple SEBORRHEIC keratosis , she has Leser Trelat sign which is a sudden onset and suggestive of underlying carcinoma of the GI tract.

Pt comes in with complaints of her groin area feeling like velvet and appearing dark in color. What is this disease associated with?

A

Acanthosis Nigricans associated with insulin resistance or malignancy especially gastric carcinoma.

Pt comes in with an ulceration on the upper lip surrounded by blood vessels. When describing it to you, she mentioned it looks like her grandmother’s pink pearls. What would you see on histology? How would you classify it? What are the risk factors and how would you treat her?

A

Patient has BASAL CELL CARCINOMA which is a malignancy of the basal cells of the epidermis. Histology shows basal cells with peripheral palisading. Risk factors are UVB, albinism and XP. Tx is surgical extension. Mets are rare.

Pt comes in with an ulceration on the lower lip. When describing it to you, she mentioned it is nodular.How would you classify it? What are the risk factors and how would you treat her?

A

Patient has Squamous cell carcinoma which is a malignant proliferation of squamous cells characterized by formation of keratin pearls. Risk factors are UVB, albinism and XP. Additional risks are immunosuppressive therapy, arsenic exposure and chronic inflammation e.g scars from burns Tx is surgical extension. Mets are rare.

Keratoacanthoma which presents as a cup shaped tumor filled with keratin debris.
