Clinical Features of Depression

what is the ICD-10 Criteria for diagnosis of depression
Set 1
- abnormally depressed mood, unchanging day to day, present for most of the day and every day for 2+ weeks
- Anhedonia for activities that are usually pleasurable
- Low energy/increased fatiguability
Set 2
- Low confidence/self esteem
- Excessive/inappropriate guilt
- Recurrent suicidal thoughts/behaviour
- Low concentration
- Appetite dysfunction (low/high)
- Sleep Disturbance
- Psychomotor Agitation/retardation
Mild = 2 from set 1, 4 total (min) Moderate = at least 2 from set 1, 6 total Severe = all from set 1 and 8 total +/- psychotic symptoms
What is the framework for depression treatment
Bio-Psycho-social approach
What are the pharmacological interventions available + when should they be considered
Chronic Mild depression (>2 weeks + all moderate/severe should be offered antidepressants
SSRIs are 1st line
Other choices include TCAs, NARIs, SSNARIs, MAOIs
Treatment resistance = lithium/another antidepressant
What is another intervention option for severe and treatment resistant depression
Electro-convulsive therapy
What indicates another SSRI should be used after commencing an SSRI
