Common side effects of all SSRIs
sexual dysfunction, drowsiness, weight gain, insomnia, anxiety, dizziness, headache, and dry mouth
Mirtazapine
Alpha 2 adrenergic antagonist (TCA) = increased serotonin and NE.
High H1 affinity
Good for insomnia or eating disorder
Biggest side effect: Drowsiness, sedation, weight gain
Sertraline
PPD, Pregnancy
MAOIs
3rd line for persistent depression
Selegiline
Irreversibly block monoamine oxidase, increasing serotonin, norepinephrine, and dopamine
Patient must avoid tyrosine in food. Fermented foods and cheeses
SNRIs prolong QTc?
NO!
TCAs
3rd line for depression
Amitriptyline
Side effects: QTc prolongation and heart arrythmias
Bupropion
Contraindicated for seizures or eating disorder or on MAOI.
Good for smoking cessation or if they need energy. No sex dysfunction.
Blocks DOPAMINE and NE reuptake
Side effects: Lowers seizure threshold, worsens SI initially
Paroxetine
SSRI
Most sedating
Fluoxetine
SSRI
Only agent approved by the Food and Drug Administration for bulimia
Longest half life of SSRIs
Serotonin Syndrome
Fast, hot, mydriasis
MCC by SSRIs
Treat with Benzos first
Cyproheptadine second
Venlafaxine
SNRI
Can cause HTN
Duloxetine
SNRI
Great for pts with significant fatigue or neuropathy pain
