Dental Implants and Their Placement - II Flashcards
What preoperative procedures should be done before implant placement?
Antibiotic prophylaxis
NSAIDs
Considerations for sedation
Chlorohexidene rinse (30 seconds right before surgery)
What is the general surgical protocol for implant placement?
Crestal incision
Full thickness flap elevation
Penetration of cortical bone with a round bur
A hole is drilled approx 6mm deep
X-ray taken with a parallel pin to evaluate drilling direction
Ostectomy is finalized to final length and diameter
Implant is torqued in manually with ratchet or by using a handpiece
When is the first post-op visit and what occurs during it?
7-10 days following implant placement
Check for signs of infection
CHx use and review of OHI with ultra-soft post-surgical treatment
Remove sutures (if necessary)
If healing is WNL, reschedule for 2-4 weeks
When is the 2nd post-op visit and what occurs during it?
1 month post-op
Check for infection
If implant was placed with 2-stage surgical protocol, make sure that the implant is still covered
If a 2-stage surgical protocol implant becomes partially exposed, what should you do?
Schedule the uncovery procedure earlier than planned
What should a surgical guide (stent) mimic?
Contours
Vertical axis
Emergence profile
What type of flap is used for implants?
Full thickness
How far should the implant shoulder sink in?
Approximately 2mm form the adjacent CEJ
What is the minimum buccal bone thickness needed for single implant placement?
2mm
What is the minimum IP bone thickness needed for a single implant?
2mm
How do you prepare a surgical guide?
Guiding canal should be prepared by using a parallelometer
On the day of crown delivery, the surgical guide can be used for implant verification
How should you treat a site with a localized dehiscence/fenestration deficiency?
Place implant and simultaneously perform lateral bone augmentation procedure
How should you treat a site with an extended horizontal deficiency
Use a staged surgical approach - bone augmentation first, and then implant later
Buccal-lingual placement of an implant
It needs to be far enough lingually so we can hide the metal-ceramic work, but not so far lingual that it causes a cantilever
How is retention gained for dentures retained by independent implants?
Ball attachments
