malocclusion (traumatic, when other options not appropriate, invasive morbidity and loss of tooth)
persistent deciduous teeth (permanent counterpart is present, leaving will cause malocclusion, crowing, PDD)
other: gingivostomatitis, chronic stomatitis, complicated crown fractures, oronasal fistula, dentoalveolar trauma some cases, caries
