Dementia with Lewy bodies Flashcards
1
Q
Definition
A
- a neurodegenerative disease involving lewy bodies which are protein deposits within neurons
2
Q
Pathophysiology
A
- normally neurons contain a protein called Alpha - synuclein
- in Lewy body dementia, this protein gets misfolded within the neuron
3, misfolded proteins aggregate to form Lewy bodies within the neuron - as disease progresses, more and more neurons accumulate lewy bodies and die
3
Q
Early symptoms of dementia with lewy bodies (cognitive)
A
( similar to Alzheimer’s )
- difficulty focussing
- poor memory
- visual hallucinations
- disorganised speech
4
Q
Later symptoms ( motor )
A
( similar to parkinson’s disease but milder )
- resting tremor
- stiff and slow movements
- reduced facial expressioms
- shuffling gait
- rigidity
- Bradykinesia
5
Q
main features of lewy body dementia
A
- progressive cognitive impairment
- parkinsonism
- visual hallucinations (other features such as delusions and non-visual hallucinations may also be seen)
6
Q
lewy body dementia + other diseases
A
The relationship between Parkinson’s disease and Lewy body dementia is complicated, particularly as dementia is often seen in Parkinson’s disease. Also, up to 40% of patients with Alzheimer’s have Lewy bodies
7
Q
diagnosis
A
- based on pattern of clinical symptoms
2. DaT scan - 90% sensitivity
8
Q
Management
A
- cognitive symptoms - acetylcholinesterase inhibitors
- Donepezil
- Rivastigmine - Motor symptoms - drugs used to treat parkinsonims
- Levadopa
