Criteria for diagnosis of Alzheimer’s
Deficits in 2 or more areas of cognition Worsening of memory and other cognition No consciousness issues Onset usually after 65 Absence of other systemic or brain diseases
Supporting findings for Alzheimer’s
Progressive cognitive functions…Aphasia, Apraxia, Agnosis
Impaired activities in daily living and behavioral changes
Family history
Normal L.P.
Normal EEG
Progressive atrophy on MRI or CT
Memory deficits
Normal general cognition and daily activities
Pre-Alzheimer’s
Mild Cognitive Impairment
Criteria for Vascular Dementia
Dementia (loss of 2 cognitive areas)
CV disease defined by focal neurologic findings
Evidence of relevant CV disease on brain imaging (infarcts)
Multiple basal ganglia/white matter lesions, etc.
Rapidly-progressing Alzheimer’s symptoms
Parkinsonian symptoms
Psychotic symptoms
VERY sensitive (bad) to anti-psychotic agents
Diffuse Lewy Body Disease
Alzheimer’s vs. Lewy Body Disease
LBD - more rapid progression, unexplained episodes of confusion and delirium (for days to weeks), Parkinsonian symptoms, psychotic, sensitive to anti-psychotic agents
Loss of memory Lack of spontaneity Difficulty in thinking or concentrating Speech disturbances Emotional dullness Loss of moral judgment Progressive dementia Fronto-temporal atrophy
