what is a chronic wound
more than 6 weeks

what is the purpose of debridement
-remove nectroic tissue
-remove bacteria to prevent infection
-increase ability to assess wound bed
what are types of non-selective/mechanical debridement
-wet to dry
-irrigation
-hydrotherapy
-abraded technique
good for mod-extensive necrotic tissue
potential for more bleeding
what is autolytic debridement
assists bodys cells in digesting nectroic tissue
-most conservative
-most ideal for noninfected wounds
-requires moist environmenta nd good immune ssystem
-takes a few days
what does honey do
-decreases pH=healing benefits
-can cause stinign
-high osmolarity which draws fluid from deeper tissue to surface
talk about enzymatic debridement
-not for heavily infected
-people can be extra sensitive to ingredients
-cant be used with silver
-expensive
talk about biosurgical debridement
maggots ew
appropriate for large wounds
-maggots eat bacteria, produce ammonia to increase pH and eat nectroic tissue
-expensive
what are some contraindications to debridement
-stable heel eschar
-gangrene
-uniidentified structures like a fungating wound or nerve/tendon etc
-terminally ill
-arterial insfuficency (ABI <0.8)
-coagulopathy
-pressure for 10 min
-elevate area
-silver nitrate sticks
-if arterial, get doctor
what to do before doing sharp debridement?
- MD order
- chart review +labs
- prep
- cleanse
- measure
- debride
