When do you start CPR?
Anyone who is:
- unresponsive
- no pulse/not breathing normally
- no signs of obvious death
When don’t I start?
- obvious signs of death
- hypostasis
- rigor mortis (stiff)
- decomposition
- danger
What are the injuries incompatible with life?
Decapitation Cranial and Cerebral destruction Hemicorporectomy Incineration Fetal Maceration (still born)
What are the lawful directions on knowing when you can’t do CPR?
Verbal from GP at the time
Written direction from actual GP
Verbal decision by health attorney
A valid health directive regarding patients wishes from the GP
When can you stop?
General Discontinuation Criteria
After 20 mins of continuous resuscitation
ROLE criteria satisfied
Asystole or PEA rate of <10
Refractory VF
When can I stop?
Rapid Discontinuation Criteria
Patient is unresponsive and pulse less for at least 10 mins prior to paramedic arrival
No CPR prior to paramedic arrival
Role criteria satisfied
Asystole
ROLE criteria is what?
Recognition of life extinct criteria
What does ROLE mean?
No palpable carotid pulse No heart sounds heart for 30 sec No breath sounds for 30 secs Fixed & dilated pupils No response to centralised stimuli
CPR with a pulse?
Must be unconscious and not breathing normally
Cardiac arrest
OR
Unconscious, inadequate perfusion with heart rate under 40 (adults)
What is does ROSC mean?
Return of spontaneous circulation
What is ROSC?
Return of any signs of life
Pulse
With or without
Some respirations
Increasing level of consciousness
When does CPR commence until?
HR above 40 Respiration rate 8-12 SPO2 above 94% High flow O2 (Regular vital signs) Posture 12LD ECG/ETCO2/Adrenaline Make sure backup is on the way Start planning extrication
