A

Infection by

  • Staphylococcus
  • Streptococcus

RF: nasolacrimal duct obstruction

Pathology

A
  1. Acute
    - inflammation
    - abscess formation
  2. Chronic
    - mucocele formation

**both will cause drainage problem

Cx:

i. conjunctivitis
ii. preseptal cellulitis

Clinical Features

A
  1. Symptoms
    i. inflammation (pain, swelling, lumpy)
    - at the medial canthal area
    ++if mucocele (painless)
    ii. obstruction (epiphora)
  2. Signs
    - inflammation (redness, edematous, tender)

Treatment

A
  1. Therapeutic
    i. antibiotic
    ii. Incision and drainage or
    iii. Darcocystorhinotomy (DCR)
    - conventional (open) or
    - endoscopic or
    - endolaser
    * creating anastomosis between lacrimal sac and middle nasal meatus
  2. Symptomatic
    i. analgesic
    ii. local warm compression
  3. Prophylactic
    i. lid hygiene (from cx)
    ii. Antibiotic
