1
Q
In D transposition, the AO arises ____ from the ____ ____ and the PA arises ____ from the ____ ____.
A
anteriorly, right ventricle
posteriorly, left ventricle
2
Q
Why is this defect so fatal?
A
Because oxygen rich blood is only filtered through the lungs and venous blood is repeatedly filtered systemically.
3
Q
Measures taken at birth to correct this defect:
A
- create ASD
- Keep PDA open
- Infant immediately intubated
- Surgery
4
Q
Surgical procedure for TGA
A
- Pallative ASD, VSD, keep PDA open
- Jantene Procedure: Arterial switch and reconstruct LVOT
- Coronaries reattatched
- ASD and VSD closed
- PDA ligated
5
Q
Cause of TGA
A
- Conal and Truncal swellings grow in the wrong direction and in the wrong position
- AO did not line up with the LV
- Truncus doesn’t rotate correctly and does not line up with the ventricles
6
Q
The only mixing of blood from the right to left side is via:
A
ASD, VSD and PDA
7
Q
Clinical features of D-TGA
A
- Hypoxic state
- Respiratory distress and failure
- HR increases
- Vessels dilate
- Increase right heart pressures (because RV is pumping systemically)
