What is the primary defect in tetralogy of fallot?
A
Anterior deviatoon of the infundibular septum (muscular septum separating the aortic and pulmonary outflow)

What are the four characteristics components of TOF?
A
- large subaortic VSD
- overriding aorta
- RV outflow tract obstruction (infundibular pulmonary stenosis)
- RV hypertrophy

What are the manifestations of TOF?
A
Cyanosis
Hypoxic spells
Easy fatigability

Clinical features of TOF
A
- cyanosed (depends on severity of pulmonary stenosis)
- retardes growth, but not mental devt
- clubbing
- squatting (lessens R to L shunting by increasing systemic vascular resistance; RV outflow tract relaxes, more blood oxygenated to the lungs)
- cyanotic spells
- hyperpnea
