A
  1. mcc of congenital cyanotic HD in children
  2. Symptoms may start after few months of birth
  3. SQUAT to ↓Rt to lt shunt
  4. Cyanotic spells
  5. Dyspnoea on exertion
  6. TET SPELLS - paroxysmal hypercyanotic attacks
    ●Activity … cyanosis, gasping, syncope
    ●Mx- Knee to chest position, O2, morphine, B-blocker
  7. Ejection systolic murmur (OR) Only S2

A
  1. Mcc cyanotic HD immediately after birth
  2. > > in mothers with DM
  3. Aorta arises from RV
    Pulm artery from LV .. immediate cyanosis
  4. No murmurs
  5. If alive = PDA or foramen ovale +
  6. Rx - PGE1 to keep PDA
    Atrial switch sx

Hypoplastic left heart syndrome

A
  1. Atresia of LV, MV, aortic valve, ascending aorta
  2. RT heart takes care of pulm and systemic supply
  3. Foramen ovale/ASD+ … mixing of blood .. PDA
  4. Cyanosis +/- till 3-4 days
    Dxt: if ductus arteriosus closes (in 3-4days) …severe cyanosis
  5. RV hypertrophy … RHF

ECG - RVH, RA enlargement, decreased lt sided forces

In >12months

  1. Soft - low amplitude
  2. Sweet - not harsh
  3. Systolic
  4. Single - no clicks/gallops
  5. Short duration
  6. Sensitive - changes with position
  7. Small - small area over chest, non-radiating

4 Criteria to dxt:
1. No symptoms
2. No fam h/o cardiac structural abn
3. No abn physical findings
4. Auscultation murmur - 7 S

CXR, ECG - cannot help
Refer to cardio > ECHO
