1st degree heart block
Normal HR, Normal P, Normal QRS, PR interval prolonged (>1 large square or 0.2 s)
2nd degree heart block
Normal HR, Normal P & QRS shape, but some QRS not preceded by a P
3rd degree heart block
P wave rate much faster than QRS rate…. No relationship between P & QRS
Premature atrial contraction
Normal P & WRS except for once early P wave that looks different from the others with a QRS to follow
Atrial tachycardia
Very rapid regular rhythm, Normal P & QRS
Junctional rhythm
Regular rate, NO P waves visible (or some inverted), Normal QRS
Atrial fibrillation
NO P waves (often the baseline is just a mess, or else nearly flat), Normal QRS shape but IRREGULARLY IRREGULAR rhythm
Premature ventricular complexes
For most beats, P waves before QRS at regular rate… Occasionally, EARLY WIDE QRS without preceding P
Ventricular tachycardia
Very FAST & REGULAR rhythm, NO P wave, Wide QRS
Torsade de Points
Rapid rate, NO P wave, Wide QRS, SINUSOIDAL pattern…..
form of ventricular tachycardia often associated w/long QT interval —
CONCERNING
Ventricular fibrillation
NO P wave, NO QRS, Irregular baseline (just a big scribble)
Atrial flutter
Rapid & regular P waves (300/min) = “saw-tooth” (very regular repeating shape)….. Normal QRS rate (100/min)
Supraventricular tachyarrhythmias w/ a regular rate
Sinus tachycardia, Reentrant SVTs (AVNRTs, ARVTs…), Focal atrial tachycardia, Atrial flutter
Supraventricular tachyarrhythmias w/ an irregular rate
Multifocal atrial tachycardia, Atrial fibrillation
Sinus tachycardia - atrial rate & P wave
Atrial rate = 100-180….. Normal P waves
