CV - tools Flashcards
what is CHA2DS2VASc used for?
to assess the pt stroke risk in AF
what is ORBIT tool used for
to assess bleeding risk in pt with AF when considering starting AC, and when reviewing pt already taking AC
offer AC with DOAC to people with AF who have CHA2DS2VASc score of …
2 and above
consider DOAC for men with AF and a score of 1, taking into account risk of bleeding
patient is under 65 with AF and they are male. another is female. would you offer AC?
Do not offer stroke prevention therapy with anticoagulation to people aged under 65 years with AF and no risk factors other than their sex (that is, very low risk of stroke equating to a CHA2DS2VASc score of 0 for men or 1 for women).
summarise CHA2DS2VASc score tool which is used to assess pt stroke risk in AF
Congestive HF/LV dysfunction (aka HF with REF etc) = 1
Hypertension = 1
Age 75 or over = 2
DM = 1
Stroke/TIA = 2
Vascular disease (e.g. Px MI, PAD, aortic plaque) = 1
Age 65-74 = 1
Female = 1
how often should you reassess pt stroke risk using cha2ds2vasc tool and bleeding risk using orbit tool?
at least annually
what is the main use of ORBIT tool
identify people at high risk of bleeding to help guide decisions on AC
orbit score ranges from ….
0-7
high risk of bleeding, medium risk and low risk of bleeding - orbit score
scores range from 0-7
0-2 = low
3 = medium
4-7 = high
what is QRISK tool used for
assess pt 10 year CVD risk
how often should you use QRISK to assess pt 10 yr CVD risk (unless they already have CVD or at high risk, or people 85 and over)
every 5 years
the following patients are considered high risk of developing CVD so CVD risk assessment using QRISk is not needed
T1D
CKD stage 3,4,5
if someone has an estimated 10 year CVD risk of 10% or more and lifestyle interventions have not been effective, what should you offer
statin - atorv 20mg is recommended for primary prevention
a patient has an estimated CVD risk of 10% or more. what do you do
lifestyle interventions
if this doesnt work, 1st line primary prevention is with atorv 20mg
