cryotherapy Flashcards
what are examples of conduction for cryotherapy?
ice massage, cold packs, ice bath, cold water circulation cuffs
what is an example of convection?
cold whirlpool
what is an example of evaporation?
vapocoolant spray
what is cryokinetics
combination of cold and exercise
What does CIVD stand for?
Cold Induced VasoDilation
what is the aka for CIVD?
hunting response- hunting lewis response
what is CIVD?
increase in tissue temp during cold therapy
approx 15 min into tx and then alternating
what are the goals for cryotherapy?
relieve or decrease pain
decrease blood flow and metabolism
protect injured tissue
decrease muscle spasm
what are the contraindications for cryotherapy?
impaired cold sensation cold-induced utricaria (cold allergy) Raynaud's disease cryoglobulinemia paroxysmal cold hemoglobinuria open wounds peripheral vascular disease confused and unreliable patients
what is an allergic reaction to cold exposure which can produce hives, itching or difficulty breathing and loss of consciousness
cold urticaria
what is an abnormal clumping of plasma proteins stimulated by cold application which leads to skin discoloration and dyspnea?
cryoglobinemia
what is cryoglobinemia associated with?
multiple myeloma, macroglobulinemia, chronic liver disease, infections, SLE, sjogrens
when cold temperature activates antibody to RBCs causing lysis, the excess hemoglobin is excreted in urine, this is called?
cold induced hemoglobulinuria or paroxysmal cold hemoglobulinuria
what are certain risks that need to be considered with cryotherapy?
impaired circulation thoracic area in patients with CAD HTN cardiovascular and cardiorespiratory disorders superficial peripheral nerves hemiplegic very young and very old obesity
What is the acronym for stages of cold sensation?
CBAN Cold Burning Aching Numbness
chemical cold packs are only used for?
emergencies
what are tests to do before application? (3)
cold/warm sensation
ice cube test
nail bed test
how full do you fill the bag with ice and where do you tie the knot?
1/2 full
tie at the top once air is removed so you can spread it out
treatment time for ice bags?
15-20min
treatment time for gel packs?
10-20 min not as effective, need a towel to improve conduction and prevent frostbite
what is the temperature for a cold whirlpool?
50-50 degrees F
what are the disadvantages of a cold whirlpool?
part can’t be elevated
not as ideal for acute situations
what is the term for a warm layer around the body part in a cold whirlpool and how do you correct it?
thermopane
moving the water around
treatment time for ice bath?
8-20 min
treatment time for ice massage?
5-10 min
10 min max
indications for vapocoolant spray
trigger points
muscle spasms
decreased ROM
precautions for vapocoolant spray
can cause frostbite
ethyl chloride is extremely flammable and shouldn’t be inhaled
contraindications for vapocoolant spray
allergy open wounds post surgical eyes all other cold contraindications
cool and rewarming technique indications
HA/ migraines sprain/strain contusions tendinitis MS pain spasm acute injury chronic and inflammatory arthritis
how does cold therapy affect pain?
decrease nerve conduction velocity
bombard CNS with cold signals-counterirritation
how does cold therapy affect spasms?
affects muscle spindles and golgi tendon organs
decreased efferent signals
interrupts pain/spasm cycle
how does cold therapy affect metabolic rate?
decreases metabolism
decreased blood demand
longer survival of hypoxic tissue
less 2ndary injury
what is the term for active exercise during or immediately following cryotherapy?
cryokinetics
what are the steps needed for cryokinetic therapy?
apply cold until numb (15-20 min) ROM and stretching for 3-5 min re-numb tissue 3-5 min continue exercise repeat up to 5x stop if painful
