1. Ischaemic rest pain for more than 2 weeks (requiring opioid)
2. Presence of ischaemic lesions or gangrene
3. ABPI <0.5

Weak or absent pulses

Spinal stenosis (pain from the back radiating down the lateral aspect of the leg, SSx relieved by sitting)
