CN II–Optic
visual acuity, visual fields, ocular fundi
CN III–Oculomotor
pupillary reactions (constriction, dilation)
CN IV–Trochlear
extraocular movements
CN V–Trigeminal
corneal reflexes, facial sensation, jaw movements
CN VI–Abducens
lateral eye movement
CN VII–Facial
facial movements
CN VIII–Vestibulocochlear
hearing
CN IX–Glossopharyngeal
swallowing, rise of palate, gag reflex
CN X- Vagus
swallowing, rise of palate, gag reflex
CN XI- Accessory
movement of trapezius muscles
CN XII- Hypoglossal
voice and speech, tongue movement (with V, VII, X)
CN I test
test sense of smell with familiar and nonirritating odors (not ammonia)
CN II test
visual acuity
visual fields by confrontation
opthalmoscope to examine optic fundus
CN III test
palpebral fissures are usually equal in width
check pupils for size, regularity, light reaction, accommodation
assess extraocular movements by cardinal positions of gaze
assess for nystagmus
CN IV test
palpebral fissures are usually equal in width
check pupils for size, regularity, light reaction, accommodation
assess extraocular movements by cardinal positions of gaze
assess for nystagmus
CN V test
Motor function: palpate temporal and masseter as clench teeth; try to separate jaw by pushing on chin
Sensory function: cotton ball touch, corneal reflex
CN VI test
palpebral fissures are usually equal in width
check pupils for size, regularity, light reaction, accommodation
assess extraocular movements by cardinal positions of gaze
assess for nystagmus
CN VII test
mobility and facial symmetry–smile, frown, puff cheeks, show teeth, close eyes tightly (against attempt to open them), raise both eyebrows
CN VIII test
assess hearing acuity
if loss present, Weber (lateralization) and Rinne (air and bone conduction)
CN IX test
Assess person’s ability to swallow, and the voice for hoarseness or a nasal quality
say “ah” and watch soft palate and pharynx
midline uvula
gag reflex by lightly touching back of throat
CN X test
Assess person’s ability to swallow, and the voice for hoarseness or a nasal quality
say “ah” and watch soft palate and pharynx
midline uvula
gag reflex by lightly touching back of throat
CN XI test
shrug both shoulders against pressure
turn head side to side against hand
CN XII
listen to articulation of person’s words
stick out tongue and move it from side to side (note any asymmetry, atrophy, or deviation from midline)
