verifies physicians order
scans chart for diagnosis & other pertinent data
x-ray, CF, bronchiectasis, etc
verifies no relative or absolute contraindications exist
modifies procedure accordingly
coordinates therapy before meals/feedings
or 2 hours after feeding
coordinate with pain medication administration if applicable (may cause pt. pain)
gathers equipment (percussor if needed)
also suction equipment, NC, NRB, ordered bronchodilator if needed
utilizes & maintains standard precaution as required
identifies pt., self, department
explains purpose of procedure
assesses vitals before therapy (HR, RR, BS, SpO2, color)
inspect chest wall for incisions, monitoring leads, IV tubing, or other possible interfering equipment
DO NOT PERCUSS ON BARE SKIN
use hospital gown, blanket, towel for pt. comfort
DO NOT PERCUSS OVER BONE, INCISIONS, JEWELRY, BUTTONS, OR BELOW RIBS
places pt. into proper positions using correct body mechanics
allows pt. to assist in turning, if able
utilizes appropriate percussion/vibration methods
maintains pt. safety at all times while turning
monitors pt. t/o tx for distress, discomfort, pain, dyspnea, arrythmias, breathing pattern, ICP (if monitored), sputum production, skin color
instructs pt. to sit up & cough with position changes
observe sputum production noting amount, color, consistency
allows periods of rest/recovery time
places pt. back into safe & comfortable position
assesses pt.’s tolerance/distress post therapy
HR, RR, BS, SpO2, color
thanks pt. & ensures needs
disposes PPE w/g
evaluates, recommends alternative procedures as applicable (PEP therapy, check oscillating vest)
documents
how they tolerate where you percussed & post vitals
