What is cardiac rehab?
a comprehensive exercise, education and behavior modification program designed to improve physical and emotional condition of patients with heart disease
What are mortality benefits from cardiac rehab?
20-30% in all cause mortality rates
decreased mortality post 5 years participation
reduction in non fatal MI post 12 months
reduced hospitalization and use of med resources
What are other benefits from cardiac rehab?
reduces sx, imporved adherence with meds, increased exercise performance, improved health related QOL
What is cardiac rehab considered by the AHA?
Class 1A recommendation
What makes you eligible for CR?
MI/ACS, CABG, PCI (angio/stent), stable angina, heart replacement, HF (EV = 35%), lung transplant
What are other conditions insurers might also allow?
PAD, HF, cardiomyopathy
What are the cardiac rehab phases?
- inpatient
- early outpatient -first 3-6 months after event can last up to a year
3-4. long term- rest of pt life
What are major components of cardiac rehab?
Patient assessment, nutrition, weight, BP, lipid and diabetes management, tobacco cessation, psychological, physical activity and exercise plan
What are desired outcomes for BP management?
- less than 140/90 for HTN pts
- less than 130/80 for DM, HF, CKD
- less then 120/80 for systolic dysfxn
What are outcome goals for lipid management?
TC under 200
LDL under 100 or under 70 for high risk pts
HDL under 40 (male), 50 (female)
Trig under 150
What are outcome goals for DM?
FPG 90-130
HbA1C under 7%
reduces hyper/hypo episodes
What are components of exercise tolerance test?
- Assess for ischemia- anginal sx, ST changes, abnormal HD response
- Determine AC- peak Vo2 and Met levels, reason for test termination
What is usual exercise prescription?
M: aerobic and resistance training
I: RPE 11-13, HR/BP/RPP parameters
D: 30-60 mins continuously including warm up and cool down, if intermittent then 30 min total a day
F: A-5x/wk, S- 2-3x/wk
