coticosteroids Flashcards
what are 6 genomic mechanism of action of corticosteroids?
(monomer tethering, monomer, dimer) transactivation / transrepression
Gene targets decreased expression by steroids
TNFa (cytokine),
RANTES (chemokine),
COX2, 5 LOX, PLA2 (inflammatory enzymes),
ICAM1, VCAM1 (Adhesin molecules),
IL2R, TCR (Receptors)
gene targets increased expression by steroids
Annexin AI (PLA2 inhibitor),
B2-adrenoceptor,
IL1RA, IL1R2 (decoy receptor)
IkB-alpha (NFkB inhibitor)
what is the anti-inflammatory effect of corticosteroids
decrease circulating T/B/Monocyte/Eosinophil/Basophil, increase neutrophil, decrease size and lymphoid content of lymph node, increase macrophage efferocytosis and promotion of resolution of inflammation, decrease type IV delayed hypersensitivity reaction

what are endocrine-metabolic side effects of corticosteroids
hyperglycemia
muscle wasting
growth retardation (esp children)
fat redistribution (moon face, buffalo hump, truncal obesity)
hormonal disturbances
delayed wound healing, skin thinning
increased appetite, weight gain
edema
what are the side effects of corticosteroids on musculoskeletal system
osteoporosis, aseptic necrosis of femoral head, myopathy
what are the side effect of corticosteroids on eye and GIT
eye: posterior subcapsular lens cataract, glaucoma
GIT: GI bleeding, peptic ulcer (especially with NSAID)
what are the side effect of corticosteroids on nervous system
mood swing, euphoria, depression, psychosis
what are the side effect of corticosteroids on immune system
opportunistic infections due to suppression of immune cells
Name of corticosteroids
Prednisone
Prednisolone Methylprednisolone Dexamethasone
