What are the different types of hormonal contraceptive
which group of women should NOT take hormonal contraceptive only ?
- women who smokes and over 35
- women with migraine at any age with aura (light noise or moving )
- blood pressure exceeding 140/90
- cardiovascular disease
- diabetes 20yrs or more with vascular disease
- history of breast disease
- jaundice or liver disease related to pass OCP use
- active hepatitis and taking Griseofulvin
- if you are on Rifampicin , phenytoin , carbamazepine Valproic Acid
10 . younger than 16 years
when would you recommend progestin only contraceptives’ ?
- women who have migraine without aura and older that 35 yrs
- women with any type of valvular heart disease
3 . Women breast feeding between 6 weeks to six months after child birth
patient want to start on COC pills but it has been more than 5 days since the beginning of her period
must rule out she is not pregnant , she can start taking the pill however she should use a back up method like a condom for 7 days
patient want to start on COC pills but it has been more than 3 days since the beginning of her period
because its within 5 days of her menstrual she can start taking the pill immediately.
patient missed only 1 hormonal pill
take the miss pill when remember , take the next pill at regular schedule time .
Miss 2 or more Pills in any 7 days
for the next 7 days uses a back up method (condom , spermicide ) then take 1 pill at once if you have 7 or more pill take the rest as usual . how every if u have less than 7 hormonals pill and its a 28 pack pill throw away the non hormonals pills. and start a new pack of pills .
Miss 1 or more Non Hormonal pill
throw the miss pill away , take the others as usual
How do you manage the side effects with EC
Take with food to avoid nausea and vomiting
breast tenderness , headache and dizziness
will go away , but can take panadol

What’s your recommendation if the victim that asked for EC is a rape victim
Refer the patient to a rape unit , however if 72 hours has passed give EC then refer to rape unit
How does EC works
- Prevent or delay the release of egg from ovary
2 . prevent egg or sperm transport in the Fallopian tube
3 . prevent transplantation of fertilized egg
What are some of the questions that you should ask a patient to rule out pregnancy
- Do you have a baby less than 6 months ago and Is fully breast feeding with no menstrual since the baby born
2 . Did your last period start with the last 7 days
3 . Have you been using a reliable contraceptive and is consistent ?
