Conn's syndrome - mx Flashcards
1
Q
What is the screening test for Conn’s syndrome called?
A
Plasma aldosterone conc to plasma renin activity
2
Q
Explain the results of PAC:PAR
A
- PAC/PRA ratio increased
- high plasma aldosterone concentration
- low plasma renin activity
3
Q
Explain the ‘confirmatory test’ for conn’s syndrome
A
oral sodium loading test
- high sodium diet/tablets for 3 days
- 24 hour urine measurements of aldosterone
4
Q
oral sodium loading test - result in healthy indivisuals
A
- RAAS is physiologically suppressed
2. inhibition of aldosterone secretion
5
Q
oral sodium loading test - result in primary hyperaldosteronism
A
- failure to suppress aldosterone secretion in primary hyperaldosteronism
6
Q
Investigations - Imaging?
A
- Adrenal CT - initial test after biochemical tests
- check for hyperplasia/tumor - Adrenal venous sampling
7
Q
Explain the method ‘adrenal venous sampling’ and why is done?
A
- used to differentiate between unilateral adenoma and bilateral hyperplasia
- Measure plasma aldosterone concentration from right adrenal vs left adrenal vein
- unilateral disease : 4 x increase in PAC
- bilateral disease : no difference in PAC between two sides
8
Q
Tx of UNILATERAL aldosterone adenoma/carcinoma
A
- surgery - adren
