Condom / Catheter Care Flashcards
What parts of the skin do you want to assess for catheter care?
perineum, urinary meatus, thigh
What must be part of your ongoing assessment?
Skin Output/intake Emotional Support no looping of tubing/bag below bladder secure connections
What is the Regular output amount/ characteristics?
30cc/hr
ylight ellow, more concentrated in morning, sediments, odor, no blood
What should the avg intake of fluid be?
1500-2000cc/day
with catheter try 2000-2500
What are the sights at risk for Catheters?
catheter insertion
drainage bag
spigot
tube junction
How can you acidify urinine and what does it do?
cran
vitamin C
prevents bacterial growth
What should you do if bag has to be elevated and why?
clamp tubing so bacteria doesn’t slide into bladder
How often should you do peri-care on pt with catheter?
BID min 3 specialized care recommended
What are 10 ways to prevent infections with catheters?
clean sites at risk increase fluid uptake acidify urine pericare BID closed system hand-washing clamp tubing if bag elevated empty bag proper positioning of bag individual measurement containers
Four steps to emptying drainage bag?
1) Gloves
2) clean spigot
3) measurement container
4) record time, amount and characteristics
4 steps to peri/catheter care?
pericare first (soap and water)
clean catheter tubing
prevent pull on catheter while cleaning
wash proximal 10cm of catheter(1/3)
What to ensure when taping?
enough slack
What equipment is needed for removing catheter?
blue pad, sterile syringe, gloves
Steps to removing catheter? (5)
1) Grab equipment
2) drain bag
3) remove tape
4) deflate buld with syringe, get fluid until tube goes in on itself
5) withdraw catheter and pinch tubing to prevent backflow of urine
When would you obtain a specimen?
dr orders
What equipment do you need to obtain a specimen?
Sterile Container, labeld
Important info when obtaining a specimen?
wipe end of spigot
take new urine, not urine thats been in bag for few hours or more
What are types of spcimens?
routine and micro
culture
When are condom catheters used?
at night depending on pt needs?
What should you ensure when putting a condom catheter on?
1inch between penis tip and condom
What should you make sure you do when condom catheters applied?
reg assessment of skin and system
Catheterization?
allows patients urine to drain freely from bladder for collection
Condom Catheter?
Condom attached to catheter to keep it in place to collect urine, used for short periods of time
Foley/indwelling catheter?
thin, sterile tube inserted in urethra into bladder, can be left for a period of time
residual Urine?
urine in bladder after urination
What should be documented when caring for someone with catheter?
Urine - odur, amount, color, characteristic skin condition intake care provided tolerance of indv.
What should be documented when removal of catheter?
urine skin condition pt tolerance intake care provided time of procedure intactness of catheter instructions given to pt
once voids on own, time amount and characteristics recorded
