All the conditions are
Blinded, charmed, defeaned, frightened, grappled, incapacitated, invisible, paralysed, petrified, poisoned, prone, restrained, stunned, unconscious
Blinded
A blinded creature cannot see and fails any ability check requiring sight. Attack rolls against this creature have advantage, and the creatures attack rolls have disadvantage
Charmed
A charmed creature cant attack the charmer or target the charmer with harmful abilities or magical effects.
The charmer has advantage on any ability check to interact socially with the creature
Deafened
A deafened creature cannot hear and automatically fails any ability check that requires hearing
Frightened
A frightened creature has disadvantage on ability checks and attack rolls while the source of its fear is within line of sight.
The creature cannot willingly move closer to the source of its fear
Grappled
A grappled creature’s speed becomes 0, and its can’t benefit from any bonus to its speed.
The condition ends if the grappler is incapacitated
Incapacitated
An incapacitated creature can’t take actions or reactions
Invisible
An invisible creature is impossible to see without the aid of magic or a special sense, it is heavily obscured. Attack rolls against the creature have disadvantage, the creature’s attack rolls have advantage
Petrified
A paralysed creature is transformed into a solid inanimate substance (usually stone). Its weight increases by a factor of 10, it ceases aging.
The creature is incapacitated and cannot move or speak, and is unaware of its surroundings.
Attack rolls against the creature have advantage.
The creature fails dexterity and strength saving throws automatically.
The creature has resistance to all damage.
It is immune to poison and disease
Paralysed
A paralysed creature is incapacitated, and cant move or speak.
It automatically fails strength and dexterity saving throws.
Attack rolls against the creature have advantage, any successful attack on the creature within 5 ft is a critical hit.
Poisoned
A poisoned creature has disadvantage on attack rolls and ability checks
Prone
A prone creatures only movement is to crawl, unless it stands up, ending the condition.
The creature has disadvantage on attack rolls
An attack roll against the creature has advantage within 5 ft of the creature, otherwise it has disadvantage
Restrained
A restrained creatures speed becomes 0, and it cant benefit from any bonuses to its speed. Attack rolls against the creature have advantage, the creatures attack rolls have disadvantage. The creature has disadvantage on dexterity saving throws.
Stunned
A stunned creature is incapacitated and ant move, and only speak falteringly.
The creature automatically fails strength and dexterity saving throws.
Attack rolls against the creature have advantage
Unconscious
An unconscious creature in incapacitated, it cant move, speak, and is unaware of its surroundings.
The creature drops what it is holding and is prone.
The creature automatically fails strength and dex saving throws.
Attack rolls against the creature have advantage, any attack that hits the creature within 5 ft are a critical hit.
