agreed recommendations from the prescriber.

The extent to which the patient’s behaviour matches
the prescriber’s recommendations.
What is concordance?
- Does not address medicines-taking specifically
• Prescriber and patient should arrive at a harmonious
agreement (a Concord) regarding therapeutic
decisions that incorporate their respective views
• A concordant consultation, leading to shared
decision-making about a prescription can promote
adherence.
What percentage of prescribed medication is not taken as recommended?
30-50%.
What can be used to measure adherence?
Subjective measures, objective measures, proxy measures.
What are subjective measures of adherence?
Patients self-report, others.
What are objective measures of adherence?
Electronic monitoring devices, markers in a body fluid.
What are proxy measures of adherence?
Prescribing data, dispensing data.
What are the consequences of non-adherence?
Reduced health and increased costs.
What is the estimated cost of unused or unwanted medicines on the NHS annually?
£100 million to £300 million.
What are some reasons for non-adherence?
Health system/HCT factors, social/economic factors, therapy-related factors, patient-related factors, condition-related factors.
