Concepts of Infection I Flashcards
What early warning score would indicate sepsis?
qSOFA or SIRS >=2
NEWS >=5
How quickly should the sepsis 6 be delivered once sepsis is confirmed?
Within 1 hour
What kinds of fluids have been proven to work in treating sepsis?
Colloids and crystalloids
How quickly should vasopressors be given when treating sepsis?
Within 1-6 hours after onset = vasopressin, epinephrine, norepinephrine
When should culture samples be taken?
Ideally before treatment is started
What are some groups that are likely to have abnormal responses to sepsis?
Extremes of age, pregnancy, immunosuppressed
What are some infections that can present vaguely and abnormally?
Liver abscesses and subacute infective endocarditis
What is the main cause of sepsis in children?
Pneumonia
What are the main causes of sepsis in adults?
Lung infections and UTIs
What are the sepsis 6?
Deliver oxygen, fluids and IV antibiotics
Monitor urine output, measure lactate and take bloods for culture
How many cultures are needed for sepsis?
One culture needed = three needed for endocarditis
Why do large abscesses need to be drained?
No blood supply = antibiotics don’t penetrate
What kind of bacteria does gentamicin cover?
Gram negative bacteria
What are the 4C antibiotics?
Co-amoxiclav, cephalosporins, clindamycin, ciprofloxacin
What is the risk associated with the 4C antibiotics?
Alter normal gut flora = increase C.diff spread
