What are the effects of external compression?
- Increase venous and lymph circulation
- Increase tissue temperature

What are the indications for compression?
- Edema
- Prevention of DVTs
- Treatment of Venous Stasis Ulcers
- Residual shaping as a result of amputation
- Control of hypertrophic scarring
How does external compression change the pressure balance between the tissues and capillaries?
By increasing hydrostatic pressure of tissue
What are the causes of edema?
- Increase capillary permeability
- Increase blood flow
- Increase plasma volume due to salt intake
- Venous obstruction or insufficiency
- Lymphatic obstruction or insufficiency
What pressure is utilized for controlling edema with compression stockings?
30-40 mmHg
When utilizing compression what should the pressure not exceed?
Diastolic blood pressure
The functionality of the lymphatic system is dependent on what?
- Vessels, valves, nodes
- Protein concentration
- Muscle activity
- Respiration
- Pulsing blood flow
What are the contraindications for external compression?
- Heart failure, pulmonary edema
- Recent acute DVT, PE, thrombophlebitis
- Obstructed venous or lymphatic return
- Severe arterial insufficiency/arterial revascularization
- Acute trauma or fracture
- Acute local skin infection
- Significant hypoproteinemia (<2gm/dL)
What are the cautions for external compression?
- Impaired sensation and mentation
- Uncontrolled hypertension
- Cancer
- Stroke
- Superficial peripheral nerves
What are the steps for treatment for pneumatic compression?
- Rule out contras
- Remove jewelry, clothing, cover open areas with gauze
- Position with elevation and comfort
- Measure BP
- Measure circumference
- Check sensation
- Apply stockinette
- Apply Sleeve
- Attach hose
- Set parameters
- Elevate limb above heart
- Instruct patient on what they should feel and do AROM when pressure is off
- Re-measure after Rx
- Inspect skin
- Apply bandage or compressive garment
- Document treatment
What do you document for external compression?
- Type of compression
- Body part
- Parameters
- pressure, on:off time, total time
- Patient position
- Pre and post measurements
- Circumference/BP
