on poas muscle
under inguinal ligament (hyperextension)
Pudendal
S2-4 vulva and perineum
entraped during sacrospinous ligament fixation
Vascular injury management
Notify anesthesia Trendelenburg position (if not in it) Emergency conversion Hold pressure Expert help (vascular surgery, trauma surgery)
Port Site Hematoma
Foley balloon with pressure
Embolism if very large
Prevention of bowel or bladder injury
Avoid cutting trocars
Placement of an NG/OG tube
Placement of a Foley catheter
Check instruments for defects in insulation
Symptoms of GI injury
Stool/enteric contents in field or incisional (abdominal/vaginal) drainage Fevers/abscess formation Abdominal pain and distention Ileus
Enterotomy closure
Close in two layers (3-0 PGA) Full thickness interrupted suture (absorb) Seromuscular imbricating suture (silk) Tension-free repair If >50% of diameter is compromised then consider resection - Suture line should be perpendicular to the long axis of the bowel to prevent narrowing
port site hernia prevention
fascia closure when >10 mm
