Etoposide (VP-16, Vepesid, Toposar)
NS, secondary tubing
Vincristine (Oncovin)
NS, secondary tubing. Do NOT dispense for intrathecal administration.
Docetaxel (Taxotere)
NS, Taxol tubing, non-PVC bag.
Paclitaxel (Taxol)
NS, Taxol tubing. Non-PVC, non-DEHP bag.
Paclitaxel Protein-bound (Abraxane)
Empty bag, secondary tubing. Reconstitute with 20ml NS.
Pemetrexed (Alimta)
NS, secondary tubing. Reconstitute with either 4.2 or 20ml NS.
Azacitidine (Vidaza)
NS, secondary tubing. Reconstitute with 10ml SW.
Decitabine (Dacogen)
NS, secondary tubing. Reconstitute with 10ml SW. Fluid bags must be refrigerated before drug is added.
Fluorouracil (5-FU, Adrucil)
Syringe push OR empty cassette with pump tubing.
Gemcitabine (Gemzar)
NS, secondary tubing
Arsenic Trioxide (Trisenox)
NS or D5W, secondary tubing. Add drug to fluid using filter needle.
Bleomycin Sulfate (Blenoxane)
NS, secondary tubing. Reconstitute with either 5 or 10ml NS.
Mitomycin (Mitozytrex, Mutamycin)
NS, secondary tubing OR just syringe. Reconstitute with 10ml, 40ml, or 80ml SW.
Ado-Trastuzumab Emtansine (Kadcyla)
NS, Taxol tubing. Reconstitute with either 5ml or 8ml SW. Do not shake.
Bevacizumab (Avastin)
NS, secondary tubing.
