Meningococcal septicaemia
This baby is pale & clammy, and does not wake on undressing. Capil refill >5 secs, HR 160 reg, RR 44, Temp 39.3. His fontanelle is normal, throat slightly red, chest clear and adbo soft. On examining his legs closely he has 5 or 6 small purple marks which do not blanch on pressing. Mum had not noticed these.
metastasis or cauda equina cord compression
On examination this gentleman has tenderness of the lumbar spine. His legs are hypertonic with decreased power, decreased sensation of sacrum & feet and hyperreflexia. PR reveals a lack of anal tone and a firm, craggy, enlarged prostate.
admit to clinical assessment unit this could be a compressed cord caused by metastasis or cauda equina
Chicken pox
fever aches and headaches
rash anywhere
loss of appetite and feeding problem
herpes zoster (shingles)
opthalmic risk
vesicular rash in dermatomal distribution
Molluscum contagiosum
squeeze - releases white cheesy fluid
8/10 happen under age of 15
keaves by itself after 6-12 weeks but high recurrence rate
atopic eczema
commonest cause of eczema
primarly in children
most children grow out of it
it is itchy dry and red skin
in infants its on scalp and face
in adults and children on the hands, inside of elbows, backs of knees
flare ups are usually caused by triggers
meningococcal septicaemia
any non blanching rash meningococcal septicaemia until proven otherwise
dial 999, give benzylpenicillin IV/IM
ceftriaxone IV/IM
Cold sore
Herpes simplex
heals within 7-10days usually
honey crusted lesions
IMPETIGO
staph and strep cause it
gets better within 2-3wks
fusidic acid abx topical
or mupriocin abx topical
Acne
teens T-zone
Boil
red hot tender inflammatory nodule that arises from hair follicle
Antibiotic- fluclox, clarithromycin or erythromycin.
Head lice
Pediculus capitis
Scabies
very itchy rash
ink test
treat all members of household with topical insecticide
wash everything above 50degrees
Threadworms
a 7yr old complains of itchy bum especially at night
tape test
treat all household and pets with antihelminthic
