Primary amenorrhea 1
- Absence of menses by age 15 with absence of growth/development of secondary sexual characteristics
- Absence of menses by age 16 with normal development of secondary sexual characteristics
Causes of primary amenorrhea include:
extreme weight gain/loss; excessive exercise; stress from major life event; eating disorders; hypothyroidism;
Thera management for primary amenorrhea:
correction of underlying disorders and estrogen replacement therapy (stimulates development of secondary sexual characteristics if absent)
Secondary amenorrhea:
absence of regular menses for 3 cycles or irregular menses for 6 months in women who have previously menstruated regularly
Dysmenorrhea
painful menstruation
pain that occurs due to menstruation itself
primary dysmenorrhea
painful menstruation due to pelvic pathology
secondary dysmenorrhea
implantation of tissue similar to endometrium in other parts of pelvis and throughout body
endometriosis
PALM-COEIN
-PALM (structural): Polyp, Adenomyosis, Leiomyosis, Malignancy
-COEIN(other): Coagulopathy, Ovulatory dysfunction, Endometrial, Iatrogenic, Not yet classified
Vaginal diaphragm (barrier)
-may be inserted 2 hrs before intercourse and must be left in place for at least 6 hrs after
-available only through prescription
-fitted (may need to be refitted if weight gain/loss of 10+lbs
-must be replaced every 1-2 yrs
Contraceptive Sponge (barrier)
-made of polyurethane, released spermicide
-can be used for more than 1 round
-does not need to be fitted/no prescription
-less effective than other methods
-no protection against STIs
-to use, wet with water, squeeze till wet and foamy, insert
-can be inserted 24 hrs before intercourse and left in place for at least 6 hrs after intercourse; provides protection for up to 12 hrs
-should not be left in for more than 30 hrs to avoid toxic shock syndrome
