What is Coeliac Disease?
Autoimmune disorder, genetic. causes immune reaction to gluten.
Where does it affect?
Mainly SI.W
what are some non-gastrointestinal symptoms.
Fatigue
joint pain
mental health problems.
headaches
how many people affected globally?
1.4% ish
Describe a healthy intestinal mucosal cells?
3-5:1 ratio of crypts (the base of villi) to villi
column shaped villious epethelial cells.
normal amount of immune cells, no inflamation
Describe CD inestine?
Flat epethelial cells,
less villi
lots of immune cells and inflamation.
reduced absorbative capacities.
accentuated ridges, and numerous oppenings to the crupts onto the luminal surface
what happens when gluten is ingested
buckle the fuck up.
- Gliadin is modified into deaminated gliadin by tTG. its now more immunogenic.
- the APC (antigen presenting cells) picks up the gluten and (you guessed it) presents it to HLA DQ2 and HLA DQ8. this causes an immune responce via the CD4+ cells
- the CD4+ cells lead to activation of b cells, which make IgA antibodies like anti tTG, anti gliadin.
- these antibodies are measured and used to diagnose CD
how is CD diagnosed?
serological testing
sI biopsy (golden standard)
describe serological testing?
check for level and presense of antibodies in blood.
anti tTG, anti gliadin, anti endomysial etc.
What is Oberhuber- Marsh classification?
criteria designed to classify CD. applied to the intestinal biopsy.
How does CD present non gastrointestinally?
(Skin, oral, gynaecological, neurologically?)
Alopecia, pruritis
defecticve tooth enamel
early menopause, infertility,
ataxia
what are consequences of untreated CD?
Risk of osteoporisis and increased fracture risk
decreased ferility
decreased quality of life
Hodgkin’s lymphoma
What is the treatment for CD
Free from diet only. no other way.
