what are the different types of migraines and describe each one
cluster: occurs in clusters with intense pain in one eye
sinus headache: when nose is blocked
tension headache: tight band around head
migraine: unilateral throbbing pain, N&V, photosensitivity, with or without aura [flashing lights]
when must you refer someone with migraine?
unresponsive to analgesics in one day taking COC recurring headache children under 12 neck stiffness severe headache for more than 4 hours recent trauma/injury
which antiemetics can you give otc for migraines?>
prochlorperazine [buccastem]
buclizine [migraleve pink]
what age requirements are licensed for sumatriptan OTC?
18-65 yp
which ppl can have sumatriptan otc?
if previously diagnosed by doctor
if previously tried analgesics
if have an established pattern of 5 or more migraine attacks in one year
what is the dosing and dosage regimen for sumatriptan?
max number of pills in 24 hours?
take one 50mg tablet asap after migraine onset
do not take a second pill for the same attack
a second dose can be taken after 2 hours if migraine comes back.
max 2 pills in 24 hours
when would you NOT give someone sumatriptan OTC for migraine? [and would refer instead]
when they have had 4 or more attacks in a month
if the migraine has lasted over 24 hours
if their migraine symptoms have changed
if it has not been previously diagnosed by a doctor
what are the symptoms of insomnia?
difficulty initiating sleep
difficulty maintaining sleep
early morning waking
poor sleep quality
when would you refer someone with insomnia?
chronic insomnia lasting more than 3 weeks
suspected depression
under 16 yo
medication ADR eg MAOIs, phenytoin, corticosteroids
what OTC products would you give someone to help then with insomnia and what age requirement?
antihistamine
diphenhydramine [nytol]
promethazine [sominex]
age 16 and over
what are the symptoms of motion sickness?
nausea and vomiting
pale skin
cold sweats
dizziness
when would you refer someone with motion sickness?
symptoms continue after stopping travel
pt taking other anticholinergic drugs eg tricyclic antidepressants
what are the OTC products for motion sickness?
antimuscarinics for journey under 4 hours: hyoscine
journey over 24 hours: hyoscine scopoderm patches
antihistamines: cinnarzine or promethazine
what is the age requirement of cinnarizine? and promethazine?
cinnarizine: 5 yo and older
promethazine: 2 yo and older
what are hyoscine contra indicated against? [2]
glaucoma and prostatic enlargement
