What are the characteristics of PTSD?
“Person exposed to a traumatic event”
- Persistently re-experience traumatic event
- Persistent avoidance of stimuli
- Persistent symptoms of increased arousal such as difficulty sleeping, irritability, difficulty concentrating, hyperviligance
What are negative cognitions associated with PTSD?
- Persistent and distorted sense of blame of self or others
- Estrangement from others
- Markedly diminished interest in activities
- Inaility to remember key aspects of the event
What is the duration of PTSD symptoms required in order to diagnose?
More than 1 month
How is PTSD treated?
SSRIs
Cognitive processing therapy
What are specific examples of cognitive processing therapy?
Support groups
Eye movement desensitization and reprocessing (EMDR)
What should you avoid prescribing in a PTSD patient long term?
Avoid addictive Rx such as benzodiazepines
What are examples of trauma?
- Domestic/family violence, dating violence
- Community violence
- Sexual or physical abuse
- Natural disaster
- Car accident
- Sudden death
- Serious injury
- Major surgery
What particular population should clinicians know are particularly suseptible to PTSD?
Military
What is the time frame for acute stress disorder?
3 days to 1 month after trauma exposure
What is an adjustment disorder?
Development of emotional/behavioral symptoms in response to identifiable stressor
Occuring within 3 months of stressor
(eg getting diagnosed with cancer, adjusting to this)
What are the symptoms of adjustment disorders?
Depressed mood
Anxiety
Disturbance of conduct (aka being an asshole)
What are examples of somatoform disorers?
Somatic symptom disorder (pain disorder)
Conversion disorder
Pain disorder
Illness anxiety disorder (hypochondriasis)
Body dysmorphic disorder
Somatoform disorder
What are the dianostic criteria for conversion disorder (aka functional neurological symptom disorder)?
One or more symptoms or deficits affecting voluntary motor or sensory function that suggest a neurological or other general medical condition
What are some examples of symptoms associated with conversion disorder symptoms?
Paresthesias and anesthesias
Weakness
Paralysis
Pseudoseizures/psychogenic seizures
Involuntary movements
Sensory disturbances (blindness, mutism)
What are factitious disorders?
Patients make themselves ill to go see the doctor
(eg. self injected feces or saliva, bizarre or unusual symptoms)
