What are some risk factors for Child Maltreatment?
Fussy, Colicky Infant
Hyperactive
Illness
Non-biologic relationship with caregiver
prematurity
(Caregiver abuses drugs, has criminal history, mental health problems)
What is meant by Child Abuse?
A recent act of failure to act that results in death, serious physical or emotional harm, sexual abuse or exploitation, or imminent risk of serious harm.
What are the 4 main types of Child Abuse?
Neglect
Emotional
Physical
Sexual
If a child is not being provided their basic needs (physical, emotional, educational or medical) what type of abuse is this?
Neglect
*Most common cause of death of abused children
If you suspect Neglect, what are some things you need to do to investigate the matter?
Full Medical History
Psychosocial History
Complete Physical Exam
*Arrange a home visit by a social worker or home nurse or local police force*
If you are worried about child abuse what must you always do?
Report/Contact Child Protective Services
If you have a child that comes in that is social withdrawn, aggressive, is failing to thrive, has self-injurious behaviors and/or emotional disturbances – among other things what sort of abuse may be present?
Emotional Abuse
If you have a child that is being emotionally abused, what are some ways you can intervene?
If isolated/child in no apparent harm: family therapy, parenting classes
If recurrent/child in imminent harm: CHILD PROTECTIVE SERVICES

Posterior Rib Fractures
Scapular Fracture
Retinal Hemorrhages
Sternal Fracture
Cigarette Burns
Metaphyseal Lesions
What type of burn patter is common with child abuse?
Glove and Stocking Burn
Cigarette Burns
What is meant by Sexual Abuse?
Employment, use, persuasion, inducement, enticement or coercion of any child
or rape, molestation, prostitution, sexual exploitation of children (incest included)
What are the 3 types of Sexual Abuse?
Non-Touching – exposing kid to porn
Touching – Fondling/penetration
Exploitation – prostitution
What is the average age of sexual abuse victims?
9-12 years old
most likely by Stepfather, boyfriend of mother, uncles or older siblings
What are the roles of physicians when suspecting sexual abuse?
usually no physical findings
History
Document using quotations
Do PE ASAP for dysuria/bleeding/discharge
(dont touch hymen)
NO SINGLE BEHAVIOR/SIGN IS DIAGNOSTIC
Could the PE be normal on a child who is being Sexually abused?
Yes, if the victim is being “reused”, and or there is delay between incident and seeing the doctor
