Malignant proliferation of lymphocytes in blood, bone marrow
Several disorders; CLL most important
Occur only in adults
Long course; indolent but incurable
chronic lymphocytic leukemia things you must know
Small, mature lymphocytes
= small lymphocytic lymphoma
B-cell, but CD5+ (weird!)
Long but inexorable course
CLL same as
SLL
immunophenotype markers for CLL
Positive for:
B-cell antigens (CD20)
A T-cell antigen (CD5)!
Negative for:
TdT
Clinical findings CLL
Older (>40)
Asymptomatic at first
Later, organ infiltration
Infection may occur due to hypogammaglobulinemia
tx cLL
Conservative
Reduce organomegaly, infections
prognosis CLL
Counts, adenopathy, BM pattern
Mean survival: 9 years
Death usually from infection
Hairy Cell leukemia things you must know
Hairy cells
Splenomegaly without lymphadenopathy
Pancytopenia
TRAP +
Clinical findings HCL
Older (>40)
M:F = 5:1
Big spleen but no lymphadenopathy!
Pancytopenia (usually) and monocytopenia (always)
why pancytyopenia in HCL
release something that causes fibrosis - hard for normal cells to get out
cytochemistry in HCL
TRAP+
immunophenotype for HCL
Positive for B cell antigens
negative for CD5
tx HCL
chemotherapy
prognosis HCL
Good - usually 10+ years
