What are the 4 parts of clinical judgment?
- Noticing
- Interpreting
- Responding
- Reflecting
What can an LPN do?
- Everything a CNA can do
- Administer meds (Not IV meds)
- Teaching, assessment
- Changing dressings
- inserting catheters
What can UAPs do?
- take VS
- Ambulate patients
- document I&O
- Empty a catheter
- Help with ADLs
What is involved in an assessment?
Finding out what the problem looks like. This includes finding objective and subjective data
What is involved in interventions?
The actions taken after finding out the interventions. (What will be done to resolve the problem)
What are outcomes in the patient’s plan of care?
This is what you want the patient to achieve (Goals)
EX: The patient will verbalize understanding about the procedure
What is evaluation in the patient’s plan of care?
Seeing if the goal or outcome has been accomplished, what interventions were effective, and what interventions need to be changed.
What type of patient should a float nurse take care of?
A patient that has a disease process that the nurse is familiar with taking care of.
What type of patient should a new graduate RN take care of?
A STABLE patient that needs assessment, evaluation or teaching.
