class III Flashcards
amiodarone
Action: inhibits K+ currents; prolonged repol time; conduction velocity unchanged
Use: ventricular arrhythmias, recurrent AFIB and A flutter; bypass mediated PVST
Side effect: has many serious side effects including pulmonary and thyroid; highly effective in tx of cases of recurrent AFIB or A flutter; merit close monitoring of the patient
dronedarone
Action: inhibits K+ currents; prolonged repol time; conduction velocity unchanged
Use: ventricular arrhythmias, recurrent AFIB and A flutter; bypass mediated PVST
Side effect: has many serious side effects including pulmonary and thyroid; highly effective in tx of cases of recurrent AFIB or A flutter; merit close monitoring of the patient
note: amiodarone analog; has much shorter half life; no side effect with thyroid, still merits monitoring
ibutilide
Action: inhibits K+ currents; prolonged repol time; conduction velocity unchanged Use: ventricular arrhythmias, recurrent AFIB and A flutter; bypass mediated PVST Side effect: has many serious side effects including pulmonary and thyroid; highly effective in tx of cases of recurrent AFIB or A flutter; merit close monitoring of the patient note: more of a "pure" class III agent
dofetilide
Action: inhibits K+ currents; prolonged repol time; conduction velocity unchanged Use: ventricular arrhythmias, recurrent AFIB and A flutter; bypass mediated PVST Side effect: has many serious side effects including pulmonary and thyroid; highly effective in tx of cases of recurrent AFIB or A flutter; merit close monitoring of the patient note: more of a "pure" class III agent
sotalol
Action: inhibits K+ currents; prolonged repol time; conduction velocity unchanged
Use: ventricular arrhythmias, recurrent AFIB and A flutter; bypass mediated PVST
Side effect: has many serious side effects including pulmonary and thyroid; highly effective in tx of cases of recurrent AFIB or A flutter; merit close monitoring of the patient
note: also has a beta blocker activity
bretylium
Action: inhibits K+ currents; prolonged repol time; conduction velocity unchanged
Use: ventricular arrhythmias, recurrent AFIB and A flutter; bypass mediated PVST
Side effect: has many serious side effects including pulmonary and thyroid; highly effective in tx of cases of recurrent AFIB or A flutter; merit close monitoring of the patient
note: blocks the release of catecholamines from nerve after initial stimulation of release; rarely used as an antiarrhythmic
