Class 2-EBP Flashcards
Evidence based practice
bridging the gap between research and practice
How do we find answers to clinical questions?
traditional approach
-could have errors; different answers
-ask a trusted friend (could be wrong)
-use your best judgment (not for sure)
-use past experience
-use textbooks (out of date)
How do we find answers to clinical questions?
research-based approach
-identify research in the area of interest
-read and understand the research reports
-does it provide evidence to support use in practice?
The Research-Practice Gap
-much of what is known from research has not been applied in practice
-takes 17 years for research to reach bedside; 30-40% of patients do not receive care with scientific evidence
EBP is…
- individual clinical expertise
- best available clinical evidence
- patient’s values and expectations
centered around improved patient outcomes
EBP is…
“integrating best current evidence with clinical expertise and patient/family preferences and values for delivery of optimal health care”
Goals of EBP
-better outcomes for patient
-provide practicing nurses with the best evidence-based data
-achieve clinical excellence
-reduce variations in nursing care
-promote effective nursing interventions
-assist with clinical decision making
Barriers to EBP
-US nurses not ready for EBP
-low comfort level with research techniques
-no time to do literature searches
-don’t know how to read and critique research reports
-negative attitudes towards research
Nurse’s role in EBP
**“the registered nurse utilizes current evidence-based nursing knowledge, including research findings, to guide practice” **
QSEN competencies for nursing KSA
-quality & safety in education of nurses (QSEN)
-knowledge skills attitude (KSA)
Training nurses to improve healthcare and quality and safety:
-patient centered care
-teamwork and collaboration
-evidence-based practice**
-quality improvement
-safety
-informatics
^^6 core competencies
Sacred cows in nursing
EX: to infuse blood you must us an 18 gauge needle or larger
evidence: research studies show that you may infuse blood with needles ranging from 22G (smallest) to 14G (largest) without causing slower infusion rates or cell hemolysis. smaller needles cause less pain and are easier to place
Sacred cow
Ex: trendelenburg position improves hypotension
evidence: the use of this position has no cardiovascular benefit
**the trendelenburg position DOES cause significant negative effects on the pulmonary system
Why is EBP relevant to nursing practice?
-there is a gap between what we know and what we do
-nursing practice must be changed from the tradition based to science based
-QSEN states that effective nursing practice requires knowledge-skills-attitudes (KSA)
-EBP empowers nurses and expands their skills
