What is cirrhosis?
Irreversible liver damage. Histologically there is loss of normal hepatic architecture with bridging fibrosis and nodular regeneration
What is the most common cause of cirrhosis?
Chronic alcohol abuse
What viruses can cause cirrhosis?
Hepatitis B and C
What genetic diseases can cause cirrhosis?
Haemochromatosis, alpha-1-antitrypsin deficiency, Wilson’s disease
How does cirrhosis present?
Ascites, clubbing, palmar erythema, xanthelasma, spider naevi, hepatomegaly, peripheral oedema, nail changes
What is AST?
Aspartate aminotransferase
What is ALT?
Alanine transaminase
What is ALP?
Alkaline phosphatase
What is GGT?
Gamma-glutamyl transferase
What should you look for on a liver US/CT?
Hepatomegaly
What does a liver biopsy do?
Confirms clinical diagnosis
What general management should you start?
Good nutrition, alcohol abstinence, avoidance of NSAIDs/sedatives/opiates
What are the acute indications for a liver transplant?
Acute liver failure meeting King’s College criteria
What are the chronic indications for a liver transplant?
Advanced cirrhosis, HCC
How often should you screen for HCC?
Every 6 months
